WOW! did you really manage to fill out the entire form - congratulations - I know it is long but it saves you time in the long run because now we shouldn't have numerous e-mail messages bouncing back and forth between us as I try to decipher what it was that you were trying to tell me (I'm kinda braindead at times ya see!)
