Thank you once again for submitting your ancestral line to
'Kelly Kanadian Kin'

Amendments and Additions

Please use this 'form' to submit any changes or additions to your existing ancestral tables within the 'researchers' pages. (If you follow your ancestral table as I have posted it, then this amendment form may make more sense; you will be able to see exactly the type of information that I am posting to the tables.)

Your name: (a required entry)
Your email address: (e.g.: you@pal.com) (a required entry)
If this email address amends a previous one, please check this box
To verify that I am amending the correct family - who is the earliest couple on your current ancestral table?
(or individual if the spouse is unknown)? - I 'file' your messages on a text page under your name and your ancestors name, so I need to know which page to open, even though you may be amending someone further down your 'table'.
ie:    Daniel Kelly/Kelley                                                            Maria FLEMING
   (a required entry) and (spouse)
Do you maintain a website that pertains to your Kelly/Kelley (Condie/Hawthorne) line? If so, I will add the address to your ancestral table as a further resource for all researchers!

The name of your site?:

Your full web site URL:

Estimated dates are fine - we can always update your 'table' again at a later date. (Remember: If the ancestor you submit was accompanied by parents and siblings in Canada - please send in what you have on the entire family; not just 'your own' ancestor.).

Enter only the information that is to be amended or added to your table.

Your earliest Canadian Kelly/Condie/Hawthorne ancestor's full name (or the individual being amended): (a required entry)

Born in:
Date of birth:
Spouse's name:
Date of marriage:
Place of marriage:
Date and place of death:
Ancestor's parents' names (including mother's maiden name):
Date of Arrival in Canada:
Area settled in Canada:
Occupation:
Religion:
Spouse's birth, (date and place):
Spouse's death (date and place):
Spouse's parents:

Just to verify! Did the individual that you have listed in the first line above come to Canada alone; or was he/she accompanied by parents or siblings?   No   Yes
If other family members were in Canada, then we are all interested in that generation also. After, transmitting this form, please consider completing another, giving the details on this settler's parents and siblings - it does not matter if you don't have full particulars - the most basic info can make all the difference when trying to connect with another researcher; even if you have to estimate times or locations.

Please submit the amended information only, on their children as follows:
Name (surname not required); date and place of birth; date and place of death; spouse; main place of residence after marriage; occupation. You need not repeat the above headings; BUT, if you could separate the info by seim-colons (;)as above and number each child, it will be easier for me to extract the information if this form should get garbled in transit.

If you are adding additional children to the family; just list all of their information as above - if you are amending existing info, ie a date of birth, list the name and the change or addition.(There is oddles of space in the blank below, so don't worry about running out of room.)

(Please mark * beside the child that is YOUR ancestor)
